Reviive Therapy Services, PLLC has a 24-hour cancellation policy. Please notify the admin or your therapist if you are unable to attend a session at least 24-hours prior to your scheduled appointment.
All no-shows and late cancellations will result in a full session charge directly to the client at the time of the cancelled or missed appointment.

If you are using an EAP plan, please have the EAP authorization code prior to scheduling an appointment. Typically, the authorization code will be given directly to the provider from the EAP company or to the client to inform the therapist. Please contact us directly if you have any questions.
